A couple of days ago the Brotherhood of Pain got their guild hall. I have two higher level toons, "WhiskeyJack {BoP}" at 35 and "Kalam Mekhar {BoP}" at 34, and one lower level toon "Silchas Ruin {BoP}" at 21. Now the two higher lvl toons both were wearing level 30 crafted cloaks (made by our crafter) with the following stats:
Armor Bonus: +5 (AC Deflection Modifier)
Enhancement Bonus: Charisma +3
Improved Saving Throws: Acid +3
Improved Saving Throws: Cold +6
Improved Saving Throws: Electrical +3
Improved Saving Throws: Fire +6
I logged in WhiskeyJack went to Sister Dye NPC in the guild hall and changed my armor and helm colors to the guild colors. Logged him out and logged in Kalam Mekhar and changed his colors to guild colors. Then I logged him out and Silchas Ruin in and did the same thing. (Silchas Ruin was wearing a dropped PvE cloak of moderate stats). I then played Silchas Ruin for several hours. Yesterday I logged in WiskeyJack for the 1st time since the guild hall went in to play him and I noticed his ac had dropped by one, I checked his inventory and his crafted cloak was gone. I carefully checked his inventory and no crafted cloak. I then logged in Kalam Mekhar and the same thing, his ac was down by one and his crafted cloak was also gone. I checked Silchas Ruin and his non-crafted cloak was there (with the new colors). What has happened? And can anything be done? Those two lvl 30 crafted cloaks were very expensive and cost our crafter a great many xp points. Any help or suggestions from DMs would be greatly appreciated.

They got dropped to the ground is what Im assuming. I have had that happen to me before, and i wasnt paying attention and ran out of the guild hall only to realize no cloak.

I've noticed cloaks most commonly will drop on the ground following being dyed for no apparent reason, however i've had it happen on any type of dye-able equipment. It's likely associated with lag.

ive noticed when i dye something that you get alot of messages about looseing said item and times my inv gets full during it for some reasion and stuff drops so it pays to make sure you got everything befor you log

I think the problem is lag-related. As I understand it you actually lose your old cloak and get a new one, but occasionally you'll get the new one before you lose your old one. Since the new cloak must go in the cloak slot (to avoid exploits), it will drop on the ground if the old cloak isn't removed in time.
This is one reason not to play for 16 hours straight...you'll start to get rummy. Kidding...but it's good to pay attention.

Nope.. it's not lag related at all.
It's scripting related, Tsavong got it -- if you watch your server messages when you dye, you will notice that each item is given to your toon like 6 times each - i assume this is for each color (1 and 2) that is applied to each item, is done 1 at a time? (not sure why it needs to use your inventory space for this, might be safer if it used a hidden container?)
To prove this, simply try dying a toon with an empty inventory -- items will never drop, then try with a full inventory -- all the items will be piled up at your feet.

how about as a rule of thumb...stop doing stuff with a full inventory? sell some stuff you pack ratting hoard mongers.
