dying during area transitions

On several occasions i've died during an area transition, i'd be at full health and a hostile critter would be nearby, after respawning, i checked to see if i was hit...i wasn't. So, i was wondering if anyone could shed light on this.

this happened to me once, i had just activated the messenger speed thing, went through a transiton at full health, came out dead

was kinda funny, but i lost xp on respawn and couldnt use my messenger speed for another 20 minutes

This has happened to me a few times as well. Hostile mob nearby, healthy when entering the doorway, dead on the ohther side. No one hits me, I'm just dead (like perma dead too not like -3 hp)... Respawn penalty, ...

Not fun.
