bugged char in the vault

there is a char named dara waynolt in mine and several other ppls char vault. its probably bugged and needs to be tracked down. a player i know seen it on and interacted with it in a way where it was making her switch between the char's(her's and the dara). she killed dara and made her way to be killed later by something else, when she respawned her char was now the dara and she was no longer herself. anyone want to take a look into this?
