if only taking one lvl of a class can it be at 20th lvl or does it have to be 19?

If I understand correctly, you can take it as your 20th level. But, if you take (for instance) 20 levels of Fighter, then 1 level of Rogue, you'll be required to take 4 more levels of Rogue.

Something I should point out, and I hope this isn't going to mess you up. Neversummer 4 requires you to take a minimum of 5 levels in any prestige class AND Paladin and Monk classes. So regardless of where you take a level of (for instance) Monk, you'll be required to take 4 more levels.
