Was it diablo that had the ear drop of the person you killed?
Perhaps something similar could happen. An object appears in the persons inventory that indicates faction and/or name and/or level of the player killed. That way if Jack and Jill whack each other over and over, it'll look kind of obvious. These objects can then be turned in for DM rewards (glow, server announcement, title, etc.) when the DMs announce it.
Alternately, maybe something similar to "job points" can be earned for PvP, for certain level ranges.
1-5 = 1 point
6-10 = 2 points
11-15 = 3 points
16-20 = 4 points
21-25 = 5 points
26-30 = 6 points
31-35 = 7 points
36-39 = 8 points
40 = 10 points
You can earn a maximum of X points a day based on your level. (We don't believe your level 5 superdude killed your level 40 friend). These points can be used for rewards through DMs, or turned into gold or xp. Also, DMs can clear your daily total so you can rampage for profit once again.
Just thowing out ways to maybe prevent exploits.

i like the points idea and being able to convert them into xp or the like make limits on how much you can "bank" and how manny points you can earn in a day and then if someone come up with max points each day you can monitor them to see if they are doing some form of exsploit.
another idea i had was to make the xp given per PK only active during events like the NC SL event the other day give the SL guys xp for killing the NC and vice versa but only for thoes in the event. that way its totaly moderated and controled.
heres another .. make PK assingment NPCs in each faction they give a random enemy faction and a short reason for the PvP then you have X ammount of time to PK as manny of that factions toons for xp per kill CR = lvl +/- 5 depending on how near they are to the hunters lvl so killing lowbies would yield little xp but going aginst higer levels would get good xp. after the time has exspired it tallys the PKs and CR of the "event" and gives you x ammount of XP for it. we can also incorperate the tital idea into this NPC as the more "quests" you go on you get better and better titals. Supreme Defender of the Northern Coalition, Grandmaster Assassin of the Shadow Legion, Grand Warlord of the Ancient Ones, High Hammer of Ragnars Kin, Grand Warlock of the Mystral Ascendancy, War General of the Last Alliance, Wilderness Champion of The Circle. ect ect

Yeah it was a key trait of Diablo, I loved those ears. I would hold entire characters just stock of the ears of the people I had killed.. Infact some guilds required atleast 3 full mules filled of ears before you could join. hehe that was a fun game ^^
